What is the MOST IMPORTANT document to have easily available in case of a medical emergency?
It is a Medical History Form, which includes your medication details and other pertinent medical details.
Let me explain..If you are found in the street slumped over, or in your own home having had a fall or looking pale at a social gathering – it is likely that the paramedics will be called to help you.Â
They have to make a quick and immediate assessment of your needs, based on any information they can find or that you may be able to tell them.
If they take the right actions – it could SAVE YOUR LIFE.
If they get it wrong…..things may not turn out so well for you.
If you have had an incident – you are often in some shock, possibly unable to speak and confused. Even if you knew ALL the right answers – you may not be able to accurately recount them. You also cannot count on someone else, in the heat of the moment, being able to do this for you either e.g. a spouse or neighbour.
So to help – I have created an abbreviated medical history form on which you can put everything that you need, in the right order including
- any medication you are taking and why
- past medical history e.g. operations, accidents, illnesses
- your next of kin
- names of any dependants
- any medical aids that you need
- and some other basic information like who your Doctor is.
